Palliative Nurse: The Future Nursing Profession Most Needed in the Era of Global Aging

Mastering Palliative Competencies To Address The Surge In Geriatric Care Demands And Chronic Patient Quality Of Life.

Indonesia is facing a dual health crisis that is rarely realized: a surge in life expectancy coexisting with an explosion in the prevalence of end-stage non-communicable diseases (NCDs).

Data from the Ministry of Health of the Republic of Indonesia reveals that more than 1.4 million patients per year require quality palliative care services, yet less than 15% can access them. Palliative care has long been misunderstood by the general public as a form of 'giving up' or merely 'care before death.' Based on the WHO Palliative Care Factsheet, the concept of palliative care is an integrated clinical approach that aims to improve the quality of life of patients—of all ages—and their families facing the complexities of life-threatening illness. This intervention relies on early identification, precise assessment, and the management of pain, physical dysfunction, psychosocial issues, and spiritual needs.

Through recent longitudinal research, integrating palliative care from the time of diagnosis has been proven to significantly prolong patient life expectancy and optimize remaining functional capacity compared to patients receiving aggressive conventional curative therapies alone (such as chemotherapy). This global paradigm shift demands that modern palliative nurses master five core cognitive and clinical competencies:

  • Multi-Modality Pain Management: Nurses must be proficient in conducting comprehensive pain assessments using validated instruments such as the NRS, FLACC, and the Abbey Pain Scale or Pain-AD Scale for dementia or non-verbal patients. Management combines pharmacological opioid collaboration with evidence-based non-pharmacological interventions like TENS, acupressure, and guided imagery.
  • Advanced Palliative Communication: Mastering high-level clinical communication protocols, such as the SPIKES technique for breaking bad news, the REMAP method for aligning goals of care, and facilitating culturally sensitive Advance Care Planning (ACP).
  • Complex Symptom Management: High competence in managing palliative emergencies and refractory symptoms, such as refractory dyspnea, terminal delirium, cachexia-anorexia syndrome, malignant ascites, and specialized palliative wound care for cancerous wounds.
  • Culturally-Based Psychospiritual Support: Particularly in the North Sumatra region, nurses are required to understand the diverse perspectives across ethnic groups, such as the meaning of end-of-life phases in the Saur Matua tradition (Batak Toba), Karo cultural rites, Mandailing Islam, and Malay traditions, to minimize family spiritual distress.
  • Care Coordination and Service Transition: The ability to design a smooth Integrated Care Pathway, coordinate interdisciplinary teams, and facilitate a patient's seamless transition from tertiary hospital care to community home care or hospice.

The Urgency of Regional Palliative HR Mapping and Professional Career Prospects

According to the WPCA Global Atlas of Palliative Care, palliative care capacity in Indonesia remains limited, suffering from acute geographic disparities. In North Sumatra, organized palliative teams are still concentrated in Medan City, while access in satellite and rural areas like North Nias, Pakpak Bharat, or North Padang Lawas is deeply lacking. This gap creates a massive demand for thousands of trained palliative nursing positions across community health centers (Puskesmas), primary clinics, and independent hospices.

Career opportunities in this field are projected to grow exponentially alongside the implementation of hospital accreditation standards (SNARS) that mandate the availability of palliative teams in type A and B health facilities. Furthermore, the expansion of Public Health Nursing programs (Perkesmas) and the post-pandemic acceleration of telepalliative care open up a new dimension for specialist nurses to provide structured, long-distance clinical assistance to patients in remote areas efficiently.
