This sociocultural desire consistently surfaces in various surveys of terminally ill patients. Unfortunately, this fundamental right to a dignified death is frequently unfulfilled due to the absence of professional medical support at home. Through the Home-Based Palliative Care (HBPC) model and the penetration of Telepalliative Care, the paradigm of end-of-life care is undergoing a radical transformation.

Clinical Efficacy of Home-Based Palliative Care: Deconstructing Complex Symptoms and Interdisciplinary Intervention

Clinically, Home-Based Palliative Care (HBPC) is not merely providing logistical convenience for outpatients, but a structured medical management system led by trained palliative nurses and supported by a virtual multidisciplinary team (physicians, pharmacists, social workers, and spiritual counselors). Scientific evidence from a global meta-analysis in The Lancet Oncology shows that the implementation of high-quality HBPC in advanced cancer patients correlates directly with a 38% reduction in unplanned ER visits, a 45% reduction in ICU admissions during the terminal phase, and a 32% lower level of family bereavement distress.

The operational application of comprehensive HBPC integrates five essential components of management from the initial visit to the post-death phase:

  • Initial Palliative Assessment: Detecting and mapping the patient's holistic needs using validated clinical instruments such as the ESAS-r (Edmonton Symptom Assessment System-revised) and FAMCARE-2 to precisely evaluate physical, psychological, and spiritual aspects.
  • Home-Based Symptom Management: Controlling refractory pain through a subcutaneous syringe driver protocol for patients with dysphagia (swallowing disorders), managing dyspnea using a combination of the semi-Fowler position and fan therapy, and preventing secondary complications such as pressure ulcers (decubitus).
  • Intensive Caregiver Training: Structured education for primary family members regarding safe mobilization techniques, enteral nutrition management, and the ability to recognize clinical signs of active dying to prevent panic and impulsive ambulance calls.
  • Virtual Multidisciplinary Team Coordination: Palliative nurses act as primary navigators connecting the patient's clinical ecosystem through secure digital platforms, ensuring a constant and consistent care plan.
  • Structured Bereavement Follow-Up: Emotional support for families for 13 months post-patient death, utilizing the Prolonged Grief Disorder scale-13 to screen for pathological or complicated grief.

Telepalliative Care and Cross-Cultural Navigation in North Sumatra

The challenging geography of North Sumatra—stretching from the Nias archipelago to mountainous regions—often isolates rural terminal patients from access to proper pain management. In this context, Telepalliative Care innovation emerges as a crucial solution. According to a study in the Journal of Pain and Symptom Management, video-conference-based palliative consultations are proven effective for routine opioid dose adjustments, remote psychospiritual support, and shared-decision making regarding treatment de-escalation.

Nevertheless, the effectiveness of HBPC in North Sumatra heavily relies on a nurse's ability to navigate cultural barriers. In the Batak Toba tradition, for instance, explicitly discussing terminal prognosis in front of the patient is often considered taboo and believed to worsen the patient's psychological state. Palliative nurses are required to apply subtle communication strategies without violating these ancestral customs. Meanwhile, in the Muslim Mandailing community, revealing a poor prognosis must be aligned with the concept of tawakkal (trust in God), where palliative nurses actively collaborate with local religious figures to formulate a spiritual narrative that strengthens the patient's emotional acceptance.

The high disparity in accessing palliative care services across various regencies and cities in North Sumatra demands that higher nursing education institutions take tactical steps to revamp their instructional systems.
