The Implementation Of UNPRI Midwifery In Developing Gentle Birth Methods To Achieve A Trauma-Informed, Natural, And Empowering Childbirth Experience

The "Gentle Birth" philosophy is not a romanticization of the past, but a clinical manifestation based on evidence (*evidence-based*) that returns the right of birthing autonomy to the mother.

The latest Intrapartum Care guidelines from the WHO affirm that supportive care in childbirth must guarantee freedom of movement, the presence of a companion of choice, and flexibility in birthing positions. Through the elimination of medical interventions not supported by strong clinical indications, this approach is proven to optimize the biological transition of the baby to the outside world while protecting the physical integrity of the mother.

Deconstructing Medical Interventions: What Is No Longer Recommended in Normal Labor

Modern obstetric and midwifery science continues to evaluate routine procedures in the delivery room. Various recent clinical studies from Cochrane and NICE Guidelines confirm that several conventional actions still prevalent in the field actually do not provide significant benefits in low-risk labor, including:

  • Routine Amniotomy (Artificial Rupture of Membranes): Not proven to accelerate the duration of labor in normal progress, but instead actually increases the risk of cord compression and intrauterine infection.
  • Enema and Pubic Shaving: The WHO explicitly recommends the discontinuation of both routine procedures because they lack an evidence-based scientific basis for lowering infection rates, and only add to the mother's physical distress and discomfort.
  • Litotomy (Supine) Position as Standard: Forcing the mother to give birth in a supine position against the force of gravity. Upright, squatting, sitting, or lateral (*side-lying*) positions are proven to better optimize pelvic diameter (*gravity-assisted descent*), reduce pain intensity, and lower the risk of major perineal trauma.
  • Mass Episiotomy: The practice of perineal cutting without medical indication is now prohibited as a routine procedure. Episiotomy is only recommended in cases of fetal distress or shoulder dystocia, not as a preventive measure against natural tears.
  • Continuous Cardiotocography (CTG): Monitoring the fetal heart rate continuously in low-risk pregnancies is proven to significantly increase the rate of *Cesarean Section* operations without lowering the perinatal mortality rate compared to intermittent auscultation methods.

The Science Behind Delayed Cord Clamping and the Clinical Efficacy of Birth Companions

One crucial pillar in the *Gentle Birth* movement is the implementation of Delayed Cord Clamping (DCC), which is the delay in cutting the umbilical cord for 60 to 180 seconds postpartum or until the cord pulsation stops completely. During this delay period, a volume of 150-200 mL of blood flows from the placenta to the baby's body. This natural blood flow is rich in iron capable of preventing iron deficiency anemia in the baby until the age of the first 6 months, as well as supplying stem cells (*mesenchymal stem cells*) and clotting factors vital for neonatal immunity.

Besides DCC, the presence of a Birth Companion chosen independently by the mother—such as a husband, parent, or close friend—is clinically proven to provide tangible physiological impacts through randomized clinical trials (*Randomized Controlled Trial*). Continuous emotional support in the delivery room directly correlates with the acceleration of the active phase of labor (on average 40 minutes faster), a 25% reduction in the use of pharmacological analgesia, a reduction of up to 31% in cesarean section rates, and results in a much higher new-born *APGAR Score*.
